Verordnungsverhalten von Protonenpumpeninhibitoren in Pflegeheimen

Abstract

Die vorliegende Arbeit analysierte das Verordnungsverhalten von Protonenpumpeninhibitoren in Pflegeheimen, d.h. die Verordnungshäufigkeit, die Indikationsstellung und die Dosierungshöhe. Eine Sekundärdatenanalyse von Baseline-Daten der vom BMBF geförderten Cluster-randomisierten kontrollierten Studie EPCentCare zur Reduktion von Antipsychotika durch psychosoziale Begleitung wurde durchgeführt. Eingeschlossen waren 437 Pflegeheimbewohner/-innen aus Sachsen-Anhalt, Nordrhein-Westfalen und Schleswig-Holstein mit mindestens einer Antipsychotika-Verordnung und einem Medikationsreview eines unabhängigen Experten. Zur Überprüfung der Indikationsstellung wurde zwischen zugelassenen Indikationen nach Fachinformation und Indikationen basierend auf Leitlinien-Empfehlungen unterschieden. Über die Hälfte der Teilnehmer/-innen mit einem Protonenpumpeninhibitor bekamen diesen entweder ohne Indikation oder in zu hohen Dosierungen verordnet. Ärztinnen und Ärzte sind gehalten, die Verordnungen von Protonenpumpeninhibitoren zu überprüfen, um nicht indizierte Verordnungen zu vermeiden und unnötige Dauertherapien und die damit verbundenen Risiken zu verhindern. Die Anpassung und Beendigung der Verordnung wiederum muss mit der gebotenen Sorgfalt erfolgen, um die Bewohner/-innen nicht zu gefährden.

The aim of this study was to assess the prescribing behavior of proton pump inhibitor in nursing homes, i.e. the prescription frequency, the indication and the dosage. A secondary data analysis of baseline data derived from the cluster randomized trial EPCentCare for the reduction of antipsychotics through psychosocial support (supported by the BMBF) was carried out. Included have been 437 nursing home residents from Saxony-Anhalt, North Rhine-Westphalia and Schleswig-Holstein with at least one antipsychotics prescribing and one medication review. For the assessment of the indication it was differ between the allowed indications in the summary of product characteristics and the indications based on medical guidelines recommendations. More than 50% of the participants with a proton pump inhibitor have been prescribed the proton pump inhibitor without indication or with too high dosages. Physicians shall review the prescribings of proton pump inhibtors in order to prevent non-indicated prescribings and unnecessary long-term therapies and the risks linked hereto. The amendment and the completition of the prescribing must be in line with the requisite care in order to avoid any risks for the participants
